5th Vital sign monitoring is always done by the Pain Nurse-on-duty and is rendered for 3 days (or as required by the Attending Anesthesiologist/ Pain Specialists). It is usually done by thorough pain assessment using the approved Pain Measurement tools like Visual Analogue Score (VAS)/ Numerical Rating Scale (NRS), Critical Care Pain Observation Tool (CCPOT), Wong and Baker, Faces, Legs, Activity, Cry, Consolability (FLACC) and Neonatal Infant Pain Scale (NIPS).
